Case Note Guidance in HMIS
What is Case Note?
Case notes are a way for staff members to record and share notes in a dedicated space within the client record. To ensure participants information is shared with other providers that may be working with the client, case notes should generally be left as public (except where necessary to protect sensitive information).
How Often Should a Case Note be Entered into HMIS?
Case managers must complete a monthly update to assess progress towards achieving the goals defined in the Housing & Services Plan. This documentation must be placed in a HMIS Case note.
Should Case Notes be Marked Private or Remain Public?
These notes should be left as public (unless there is sensitive information included) to allow for coordination among the different providers assisting the client.
What Information Should be Excluded from a Case Note?
Do not enter diagnosis information, medical records, mental health goals, etc. as this information is sensitive and should not be entered into HMIS.
Where Should Case Notes be Entered?
Case notes should be entered as a program case note most of the time.
